Acknowledgment of Risks:
I, {name}, acknowledge and fully understand that boxing and fitness activities involve inherent risks, including but not limited to physical injuries, muscle strains, sprains, fractures, or other serious injuries, which may result from participation in these activities. I voluntarily assume all risks associated with my participation in activities at Ramos Boxing Academy.

Release of Liability:
In consideration of being allowed to participate in activities at Ramos Boxing Academy, I hereby release, waive, and discharge Ramos Boxing Academy, its owners, agents, employees, trainers, and any affiliated entities from any and all claims, liabilities, or demands for personal injury, property damage, or loss that may arise from my participation, whether caused by negligence or otherwise.

Medical Consent:
I certify that I am in good physical condition and have no medical or health conditions that would restrict my ability to safely participate in boxing or fitness activities. I agree to inform Ramos Boxing Academy immediately of any health issues that arise. In the event of an emergency, I authorize Ramos Boxing Academy to seek medical attention on my behalf.

Photo/Video Release:
I understand that Ramos Boxing Academy may take photos or videos during classes or events for promotional purposes. By signing below, I grant permission for my likeness to be used in marketing materials, social media, or advertising. If I do not consent, I will notify Ramos Boxing Academy in writing.

Membership Cancellation Policy:
Memberships at Ramos Boxing Academy are billed monthly. I understand and agree to the following cancellation terms:

To avoid being charged for the next billing cycle, I must submit my cancellation request at least 3 days before my payment date.
If I cancel my membership after the payment date, I understand that no refunds will be issued for that billing cycle.
Cancellation requests must be submitted in writing or via email.

Acknowledgment and Agreement:

By signing below, I acknowledge that I have read, understood, and agree to the terms of this waiver and release form, including the cancellation policy. I voluntarily accept the risks associated with participating in activities at Ramos Boxing Academy and agree to abide by all gym policies and procedures.
